Below, you'll find important forms for your convenience and contact information for our dedicated technology support leaders at each school.

Important Forms for Parents/Guardians:

  1. SB 20 - Acceptable Use Policy and Agreement: Review the SB 20 - Acceptable Use Policy to ensure safe and responsible use of school technology resources by your child. Complete, sign, and return the Student Agreement page to your student's school. This form is required to be on file for your student to be issued and/or access any school affiliated technology.

  1. Device Repair / Replacement Agreement : Our Device Repair/Replacement Agreement outlines the terms and conditions regarding repair and/or replacement of school-issued devices. It establishes guidelines for responsible use and care of these devices, ensuring that students and parents/guardian understand their roles in maintaining them. This agreement is required to be on file before a student is provided a school-issued device and streamlines the process for device repairs or replacements in the event of damages, ensuring minimal disruption to the student's learning experience.

  1. Optional Damage Waiver Application: The optional Damage Waiver Application allows parents/guardians to apply for coverage that protects against accidental damage to school-issued devices. By completing this application, families can opt into the program, providing peace of mind knowing that potential repair costs are significantly reduced and access to essential technology for their child's education is uninterrupted. This is an optional waiver/form.

  1. Device Damage or Loss Report: The Device Damage or Loss Report form is used to report any instances of damage or loss concerning school-issued devices. It enables swift documentation of incidents and initiates the necessary steps for repair or replacement. Submission of this report within 72 hours of an incident ensures efficient resolution, minimizing disruptions to the student's learning experience.
